Register for your sport on Genesis-

Medical Forms:
The following forms need to be printed and turned into the nurse’s office in your building in a sealed envelope with the athlete’s name and sport clearly written on the outside.
A. New Athlete Physical

Physicals are good for 1 calendar year (365 days) from the date of the first day your spring sport can practice. See the chart blow.

ImPACT testing
A. Register in Genesis for your sport before 2/10/2023
B. If you do not have a valid ImPACT test on file, you will receive an email through parent and student school email with a link. (Baselines expire after 2 years. Last test date is available on Genesis under Athletics)
C. Be prepared to test at home virtually on one of the dates and times available. This season those will be 2/22/2023 and 3/1/2023 at 4 and 5pm.
