SPORTS PHYSICALS ARE ONLY VALID FOR 365 DAYS.
IMPACT TEST IS VALID FOR TWO YEARS.
Sports forms required - Athletic Office – Once per school year
Code of Conduct, NJSIAA Steroid Policy, Concussion/Head Injury and Sudden Cardiac Death.
Winter Sports – Medical — Basketball, Ice Hockey, Fencing, Wrestling, Bowling, Swimming, Cheerleading and Winter Track
- If you played a fall sport and you are playing Basketball, Wrestling, Fencing, Cheerleading, and or Track & Field and your physical was dated after December 1, 2014, it is still valid, the only form you need to submit is the Health History Update Questionnaire.
- History Form, Supplemental Needs (even if it is not applicable), Physical Examination and Clearance Form need to be handed in. (this applies to all 9th)
- For all 9th grade students, who have not played a fall sport, you will need to submit the History Form, Supplemental Needs (even if it is not applicable), Physical Examination and Clearance Form must to be submitted, from your last valid physical. All forms must be signed.
- If the 9th grade student’s physical is dated more than 90 days from the first day of practice you must submit the Health History Update Questionnaire.
- All 10th, 11th and 12th grade students who submitted and still have a valid physical from last year, the only form you must submit is the Health History Update Questionnaire.
- Forms can be submitted on:
- October 5th for Swimming and Ice Hockey
- October 12th for Bowling, Winter Track and Wrestling
- October 20th for any Winter sport – 4:30 pm to 8:00 pm *******
- October 22nd for any Winter sport
- October 28th for Fencing and Basketball
- From 4:00 pm to 8:00 pm in the LGI of the Main Building. No forms will be accepted during or after school.
- Physicals for athletes without Primary Care Physician are going to be on:
- October 5th, October 12th and October 28th
- From 4:00 pm to 8:00 pm in the LGI of the Main Building.
